Missouri DIP Completion Form: What It Is and Where It Goes

Updated October 2026 · Reviewed by Billy Forte

The Missouri DIP completion form is the proof of completion you get after you finish a court-ordered Driver Improvement Program. Missouri law says the court must forward a record of your completion to the Department of Revenue (DOR) within 15 days after you finish. The DOR FAQ also says you or the program may mail the completion form to the DOR’s Jefferson City address.

Key Facts

  • What it is: proof that you finished the program; the law calls it a “record of the completion.” See what the form is.
  • The number: “MO Form 4444” comes from the Missouri Safety Center process sheet, not the DOR forms site. See the Form 4444 name.
  • Where it goes: the DOR FAQ lists one Jefferson City mailing address. See the address.
  • Deadlines: finish within 60 days of conviction; the court reports within 15 days after you finish. See two deadlines.
  • Checking receipt: call 573-526-2407 or email [email protected]. See how to confirm.

What the Missouri DIP Completion Form Is

When you finish a Driver Improvement Program (DIP), the program gives you proof of completion. People call it a certificate or a completion form. The paper itself matters less than the record it creates.

The law uses its own words. RSMo 302.302.5 says the court must “forward a record of the completion to the director.” The director is the Director of Revenue.

The statute does not name a form or a “certificate.” The DOR Tickets and Points FAQ calls it “the completion form.” It gives no form number.

Where the Form 4444 Name Comes From

You may see the number 4444 online. It appears on the Missouri Safety Center DIP Process sheet. That sheet says: “the MO Form 4444 must get to Department of Revenue (DOR) within 15 days.”

The DOR links to the Safety Center for its list of acceptable providers. But the number does not appear on the DOR forms site. A forms search there for 4444 returned “No results found.”

So use the name with care. “MO Form 4444” is the Safety Center’s name for the paper. The DOR’s own FAQ just says “the completion form.”

Form 899 Is a Different Form

DOR Form 899 is the DOR’s list of violation descriptions and the points each one carries. It is not a DIP form. It does not mention the DIP or Form 4444.

The DOR Mailing Address

The DOR FAQ says “you or the program may mail the completion form to the following address”:

Missouri Department of Revenue, 301 West High Street – Room 470, PO Box 200, Jefferson City, MO 65105-0200

That is the full address as the FAQ shows it. It has no bureau or office name added. Copy it exactly if you or your program mails the form.

Who Sends It

Two official sources describe this step in different ways. Here is what each one says:

  • The statute. RSMo 302.302.5 puts the duty on the court. The court must forward a record of completion to the DOR within 15 days after you finish.
  • The DOR FAQ. It says “you or the program may mail the completion form” to the address above.

The statute is the law, and the statute controls. The FAQ adds that you or the program may also mail it. Ask the court on your ticket how it handles the record, so you know what to expect.

Our guide on how completion reaches the DOR covers this step in more depth.

The path of a Missouri DIP completion record, from the court order to the DOR's points record

Who Verifies Completion

The statute says the DIP counts only when it is “ordered and verified” by the court that has the case. So the court is part of the story both before and after you finish.

Under RSMo 302.302.5, a court that uses a centralized violation bureau established under RSMo 476.385 may have that bureau order and verify completion. Your ticket names the court or bureau that has your case.

The Fine Collection Center Name

One older name still shows up on the DOR FAQ: the Fine Collections Center. The Missouri courts’ Statewide Fine Schedule page says the center “stopped accepting new citations” on May 1, 2019.

Under Supreme Court Rule 38.10, each county now has a circuit violations bureau. Do not send your completion form to the old center.

Two Deadlines, Two Jobs

Two deadlines touch the completion form. They run from different dates and belong to different people.

  • Your deadline: 60 days. The law says the course “shall be completed within sixty days of the date of conviction.” This is a deadline to finish the course. It is not a mailing deadline.
  • The court’s deadline: 15 days. After you finish, the court must forward the completion record to the DOR within 15 days.

If you miss the 60-day finish, the law does not let the DOR accept the DIP in place of points. No official source mentions an extension. See our guide to a missed DIP deadline.

What to Keep After You Finish

Keep a copy of your proof of completion. It is your own record that you finished, and it helps if a question comes up later.

It also helps to write down a few details from your case:

  • Your conviction date, which starts the 60-day window.
  • The date you finished the course.
  • The court’s name and your case number, as shown on your ticket or court papers.
  • The date and way the form was sent, if you or the program mailed it.

Two Kinds of Certificate

The word “certificate” shows up in two places, and they are not the same thing. One is yours, and one belongs to the program.

  • Your proof of completion. The program gives it to you after you finish. The court’s record of it goes to the DOR.
  • The provider certificate. Under 7 CSR 60-3.010, a designee chosen by MoDOT’s Highway Safety and Traffic Division issues it to approved programs. It lasts one calendar year.

The DOR does not approve or list programs. Its FAQ sends readers to the Missouri Safety Center website for acceptable providers. Check that list before you enroll.

Online and In-Person Courses

RSMo 302.302.5 lets you choose an online or an in-person course. Either way, the program must “meet or exceed” the National Safety Council’s eight-hour “Defensive Driving Course” standards, and the DIP completion rules are the same.

How to Confirm the DOR Got It

The DOR gives a phone number and an email for this. Its FAQ says to call 573-526-2407 or email [email protected]. The same contacts work to check how many points you have.

The DOR FAQ also lists these contacts to check whether a court order was received. Under RSMo 302.302.5, the court must forward a record of completion within 15 days after you finish. If you have questions about that report, the court on your ticket is the place to ask.

Our guide on checking points on a Missouri license explains each way to look.

What the Form Does and Does Not Do

The completion record has one job. Once the court orders and verifies the DIP, the DOR accepts it “in lieu of the assessment of points” for that one ticket.

For a speeding ticket, that means the 3 points for a state-law charge, or 2 for an ordinance charge, are not assessed. A court-certified 2 extra points for injury or damage are covered too.

The form does not do more than that:

  • The conviction stays. The Safety Center sheet says the program “WILL NOT remove the ticket from the drivers record, only the points.”
  • Old points stay. It does not remove points already on your record.
  • The fine stays. It does not lower the fine or court costs.

When No Form Will Help

A completion form cannot make an ineligible case count. RSMo 302.302.5 sets limits that no paperwork changes:

  • No court order. The DIP counts only when the court handling the ticket orders and verifies it.
  • CDL holders. No DIP for anyone who has or must have a commercial license, in any vehicle, under the DOR commercial driver page.
  • Commercial vehicles. No DIP for a violation in a commercial motor vehicle.
  • Motorcycle tickets. These need an approved motorcycle-rider training course.
  • The 36-month limit. A DIP counts no more than once in any 36-month period.
  • Offenses outside the DIP. DWI, driving while suspended, leaving the scene, no insurance and felonies are not covered.

What We Found on the Official Pages

On the Missouri DOR pages we checked on October 27, 2026, a forms search for 4444 returned “No results found.” The DOR forms list went from 4441 straight to 4458.

The DOR FAQ named “the completion form” with no number and no link. It listed the Jefferson City address shown above, with no bureau name.

The Safety Center DIP page linked to its process sheet. That sheet’s file name was dated 4-20-2026, and it was the only official-linked source for the number 4444.

Common Mistakes to Avoid

Each item below comes from Missouri law or an official page:

  • Treating 60 days as a mailing deadline. It is the deadline to finish the course, counted from the conviction date.
  • Adding a bureau name to the address. The DOR FAQ address has none.
  • Looking for 4444 on the DOR forms site. The DOR search returned no results.
  • Mixing up Form 899. That form lists violations and points, not the DIP.
  • Assuming the record arrived. Call or email the DOR to check.
  • Expecting the ticket to disappear. The conviction stays on your record.

How Requirements Can Vary

Missouri’s point values, suspension levels and Driver Improvement Program rules come from Missouri law (RSMo 302.302 and 302.304) and apply statewide. Whether a court orders the DIP for your ticket, and how your case is handled, is up to the court on your ticket. Fines and court costs depend on the court and the charge.

Laws, forms and court practice can change. Check your ticket, any court order, the Missouri DOR website and the Missouri Revised Statutes before you enroll or pay.

Frequently Asked Questions

What is the Missouri driver improvement program certificate?

It is the proof of completion the program gives you after you finish. The law calls it a “record of the completion,” and the DOR FAQ calls it “the completion form.”

Is Form 4444 a DOR form?

The number does not appear on the DOR forms site, and the DOR FAQ gives no form number. The Missouri Safety Center process sheet calls the paper “MO Form 4444.”

Where do I mail the completion form?

Use the Jefferson City address from the DOR FAQ, shown in full above. It goes to Room 470 at 301 West High Street, with PO Box 200 and ZIP code 65105-0200.

Does the court send my completion to the DOR?

Yes. RSMo 302.302.5 says the court must forward a record of completion to the DOR within 15 days after you finish. Ask the court how it handles your case.

Is there a 60-day deadline to mail the form?

No. The 60-day rule is to finish the course, counted from your conviction date. The 15-day rule is for the court’s report after you finish.

Does a motorcycle ticket use the same completion form?

No DIP is used for a motorcycle ticket. RSMo 302.302.5 calls for an approved motorcycle-rider training course instead. Ask the court how it wants proof of that course.

How do I know the DOR received it?

Call the DOR at 573-526-2407 or email [email protected]. The DOR FAQ lists these contacts for points questions and to check whether a court order was received.

Will the completion form remove my ticket?

No. The DIP keeps points off that one ticket, but the conviction stays on your record.

Conclusion

The Missouri DIP completion form is your proof that you finished, and the court must report it to the DOR within 15 days. Keep a copy, use the DOR FAQ address if you or the program mails it, and call or email the DOR to confirm it arrived. For readers whose court has ordered the DIP, you can take the Missouri Driver Improvement Program online.
